Oyster Creek Nuclear Generating Station Docket No. 50-219 IE Jnspection No. 81-11 This letter is in response to the " Notice of Violation" as stated in your letter of July 1, 1981, with regard to the inspection conducted on May 1-31, 1981.

Violation:

Technical Specification 6.8.1 states in part, " Written procedures shall be established, implanented, and mainta.tned that meet or exceed the requirenents of... Appendix 'A' of Nuclear Regulatory Cmmission's Regulatory Guide 1.33-1972..."

Regulatory Guide 1.33-1972, Appendix

'A',

lists " bypass of safety functions and jumper control" as activities for which procedures should be prepared.

Procedure 108, Revision 25, Decanber 22,1980, "Equignent Control",

states in part, "5.2.1 The Group Shift Supervisor is responsible for controlling the installation and renoval of numbered electrical jumpers..

5.2.1.1 No electrical jumper will be installed, raioved or modified without prior authorization of the Group Shift Supervisor or Group Operating Supervisor..

5.2.2 'Ihe electrical jumper check-off sheet shall be used to control the issue of all applicable jumpers..."

Cmtrary to the above, cn May 19, 1981, the following numbered electrical jmpers were fcund to be ingwperly installed or dis-cmnected:

Electrical Jmper Checkoff Sheet 79-151 dated January 12, 1979, specified junper nutber 60 to be installed between terminals 187 and 189 in the ocndensate deineralizer panel to defeat the low water flow trip to the caustic pump. 'Ihe junper was found disconnected fr m terminal 187 without authorization.

-- Electrical Junper Oleckoff Sheet 81-342 dated April 24, 1981, specified jumper number 75 to be installed between reminals 7F-TB17-36 and 7F-TB17-48 to allow operaticn of the turning gear and lift puups while performing 230 KV line modifications.

The junper was found disconnected fran tenninal 7F-TB17-48 without authorization.

- Electrical Junper nunber 14 was found installed between terminals 7F 'IB1-176 and 7F-TB184 to junper the main flash tank high level alarm. It was installed without the use of a jumper check-off sheet or any other docurrented authorization.

Respcnse:

Our imnediate corrective action, as stated in the inspection report, was to evaluate the electrical junper positions in questicn and correct any deficiencies. In addition, in order to insure additional discrepancies did not exist, our junper log was reviewed and all jumpers accounted for prior to plant startup frcm our Spring 1981 maintenance outage. Recently, we have completed a field verification of all jumpers and plan to conduct this type of inspecticn periodically as discussed below.

In order to insure the integrity and accuracy of our jumper control systen, our surveillance program will be modified to include a quarterly physical verificaticn of all electrical junpers and lifted leads. This additional surveillance will be incorporated by August 31, 1981. In addition, we will ocntinue to snphasize the importance of equipnent ocntrol practices and the associated administrative requirenents as identified in Procedure No.108 "Equippent Control" to all Operations persmnel. 'Ihe details and locations of the jumpers involved are such that attenpts to definitively detennine the circumstances and perscnnel involved could not be readily detennined. The above actions should prevent further violations in this area.

Full cotpliance was achieved by our innediate action of correcting the noted deficim cies. This was acccmplished cn May 19, 1981.
