NOTICE OF VIOLATION 4l Department of the Air Force License No. 04-17358-04 USAF Regional Hospital March I

March AFB, California 92508 As a result of the inspection conducted on July 16, 1981, and in accordance l

with the Interin Enforcement Policy, 45 FR 66754 (October 7,1980), the i

following violations were identified:

A.

License Condition 15 requires that the licensee shall possess and use h

licensed material in accordance with statements, representations and 3

procedures contained in the application dated November 1,1976 and letter dated March 7, 1977. Section I-7 of Hospital Regulation 160-12, which is incorporated into the license application dated November 1, 1976, requires that the Radiation Protection Officer's duties shall include the following:

(1) Conduct a quarterly audit related to the radiation surveys required in Nuclear Medicine work areas.

(2) Review leak tests results of sealed sources conducted by Nuclear Medicine personnel once every 6 months.

(3) Supervision of waste disposal procedures.

Contrary to these requirements, the PPO has not perfortned these duties for at least one year prior to the inspection.

These items constitute a Severity Level VI Violation (Supplement VII).

B.

10 CFR 20.401(b) requires that each licensee shall maintain records showing the results of surveys required by 20.205(b) and 20.201(b).

(1) Contrary to this requirement, records of surveys of packages receiv d containing licensed material were not being maintained.

(2) Contrary to this requirement, records of surveys of items formally \\

containing radioactive material and which was surveyed and found

to be no longer radioactive and consequently buried with non-radioactive trash, were not being maintained.

These items constitute a Severity Level VI Violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, USAF Regional Hospital March is hereby required to submit to this office within thirty days of the date of this notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
