g This refers to the routine safety inspection conducted by Mr. P. R. Zurakowski of this office on July 16, 1981 of activities authorized by NRC License No. 04-17358-01 and to the discussion of our findings held by Mr. Zurakowski with Dr. Edward Parker and other members of your staff at the conclusion of the inspection.
The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Conmission's rules and regulations and the conditions of your license.
The insper+. ion consisted of selective examinations of procedures and representative records, interviews with personnel and cbservations by the inspector.
During this inspection, it was found that several of your activities appeared to be in noncompliance with NRC requintments. The items and references to the pertinent requirements are listed in Appendix A.
These items have been categorized into severity levels as described in the Federal Register Notice (45 FR 66754) dated Octcher 7, 1980.
Your response to this notice is to be submitted in accordance with the, a visions of 10 CFR 2.201 as stated in Appendix A, Notice of Violation.
