Unit 2 "B" Feed Water Turbine Overspeed to Destruction On June 22,1981, the Unit 2 "B" feed water turbine was operated in such a manner that caused the turbine to overspeed to destruction. We have reviewed, with our resident inspectors, the events surrounding this occur-rence and conclude that although the system was non-safety mlated, the overspeed and subsequent destruction of the turbine could have randemd inoperable a safety mlated system.
In view of this, you are twiuested to respond, in writing, within 30 days giving us your evaluation of the occurrence and identifying what steps you have initiated or intend to initiate to prevent a recurrence.
