i This refers to the inspection conducted by H. W. Roberds of this office on l
Mart:h 10-13,1981, of your facility at Cleveland, Ohio, associated with the fabrication of standby gas, treatment systems and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.
This inspection was made as a result of:
(1)theissueofa10CFRPart21 l
report pertaining to the identification of defects in a temperature detection -
unit type, that had been installed in equipment furnished to the Hartsville and Palo Verde Nuclear Generating Stations; (2) the identification of incorrect connection of differential pressure gages in air clean up units furnished to l
'Bellefonte Units 1 and 2; (3) the identification of defective Flanders Filters i
charcoal adsorber trays at Sequoyah Units 1 and 2; (4) the identification of I
separation of screens from charcoal aosorber frames at the Phipps Bend and i
Hartsville Nuclear Stations; and (5) the identification of inadequate fire protection system drains in standby gas +.reatment charcoal adsorbers furnished to the Phipps Bend and Hartsville Nuclear Stations.
1 Areas examined and our findings are discussed in the enclosed report. Within I
these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and ooservations by the inspector.
During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. The specific findings and references oCE to the pertinent requirements are identified in the enclosures to this letter.
