PVMP 1 Standard Procedure for Visual Inspection of Reactor Vessel Internals for 3410 Type Plants The enclosed Standard Procedure for Visual Inspection of Reactor Vessel Internals is in accordance with the requirements of NRC Regulatory Guide 1.20. Within these

guidelines, visual examination of the internals shall be perfonned in two stages, a baseline review and a post hot functional precore test review, with msolution verified under the conditions of lighting, distance and angle specified in Para-graph IS-211.1 of Section XI of the ASME Code. The inspection NPort shall include a written description of all reportable conditions observed, such as cracks, scratches, corrosion, emsion, wear, movement or misalignment. In addition, if any such conditions are unsatisfactory or questionable in the judge-ment of the inspector, the Customer shall be imediately notified in writing. The inspector shall be qualified in accordance with Section 7.0, " Inspector Qualifica-tion Requirements", of this procedure. Verification of perfonnance shall be assured by the signature of a qualified auditor following each inspection series (see Q. A. auditor's Check Off List enclosed).

Photographic documentation of both the baseline review and post hot function (PHF) test review will be conducted and included as part of this procedure.

The inspection report will contain a tabulation of the following items:

a. Data sheets with component descriptions and photos per the procedure. .
b. Names and qualifications of personnel perfonning the inspection.
c. Date of the inspection.
d. Signature of the auditor.
e. A qualification mcord of the auditor.
f. A copy of the procedure used.

g. A detailed comparison of the baseline & PHF component's wear characteristics.
h. Identification of the reactor plant.

1-Foreword The inspection and documentation ft.,r the Reactor Internals shall be completed in two stages. The first stage shall take place prior to the cold hydradlic testing of the primary system and the se:ond stage shall take place after the hot func-tional pre-core testing of the internals is completed. The first stage inspection will compare the reactor internals components with design specifications, standards, and drawings. After the second stage inspection is completed, the data will be compared with the data from the first stage inspection and this comparison will detemine the component performance and provide a basis for a comprehensive vibration assessment.

1.0 PREREQUISITES (Baseline Series) 1.1 The internals components (Core Support Barrel Assembly and Upper Guide Structure Assembly) shall be individually positioned outside of the reactor vessel and assembly of internals must be complete. The com support barrel must be raised to allow inspection of the bottom flexure weld; and the upper guide structum shall be oositioned at least' thme feet off the floor to allow inspection of the CEA shroud to fuel align-ment plate retaining bolts and lockbars located on the bottom of the fuel alignment plate.

1.2 The reactor v6sel shall be empty and suitable for inspection of the flange seating surface, key slot surface, outlet nozzles and the vessel snubbers, positioned around its bottom head ama.

1.3 The internals components shall be clean so that all surfaces to be in-spected are free of contaminants, dust, foreign matter, etc.

1.4 The reactor vessel closure head key: slot surface and holddown ring mating surface shall be accessible for inspection and photographing.

1.5 Sufficient scaffolding shall be set up to allow complete access to com-ponents and inspection surfaces for at least three (3) persons; i.e.,

inspector, photographer and assistant. The scaffolding shall be located in the reactor vessel just below the outlet nozzles (to allew 100% nozzle access) and below the vessel snubbers (to allow all snubber surfaces to be inspected), and scaffolding shall be located across the top of the ccre shroud (to allow access to the guide lug bearing surfaces).

1. Electric outlets (110V) for standard, two-pmng plugs must be available at or near che components.

2.0 PREREQUISITES (Post Hot Functional Pm-Core Series) 2.1 The reactor shall be couled to less than 100 0F, drained and the react.cr vessel closure head renoved and seated on its storage stand with its key slot surfaces and holddown ring mating surface accessible for inspection.

2.2 The internals components (Core Support Barrel and Upper Guide Structure) shall be individually positioned outside the reactor vessel so that the bottom of the core barrel is at least three feet off the floor to Specification No. 00000-RCE-413 Revision 00

2.3 The reactor vessel shall be empty and suitable for inspection of the seating flange surface, key slot surface, outlet nozzles and the vessel snubbers.

2.4 The internals components shall be cleen so that all surfaces to be inspected are free of contaminants, dust, foreign matter, etc.

2.5 Sufficient scaffolding shall be set up to allow complete access to components and inspection surfaces for at least three (3) per-sons; i.e., inspector, photographer and assistant.
