Docket No. 99900334/81-01 NOTICE OF NONCONFORMANCE Based on the results of"an NRC inspection conducted on April 6-8, 7.981, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states:

" activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily' accomplished."

Nonconformances with these requirements are as follows:

A.

Weld procedure No.100-W-1A, revision two and weld procedure No.100-W-27, revision three list the size range of weld rod approved for use with

_ each procedure.

Contrary to the above requirements a welder was using 3/32" weld rod on job No. 96632 that had not been approved for use with the specified weld procedure, No. 100-W-1A.

On job No. 94922, another welder was using 3/16" weld rod that had not been approved for the specified weld procedure, No. 100-W-27.

B.

Paragraph 9 1 of specification No. 100-A-3, revision four, requires all weld material be returned to stores at the end of each four hour period.

Contrary to the above requirements, 3/32" weld rod being used on job No. 95395, weld No. 14 had been issued five hours previously.

C.

Weld procedure No.100-W-18, revision two, specified a maximum of 24 volts and 100 amps to be used.

Contrary to the above requirements, job'No. 95395 was being welded using procedure 100-W-18 with 29 volts and 150 amps.

Under the heading of paragraph 7.3 of the ASME accepted QA manual titled

" Welding in Process", paragraph 7.3.3 requires that all nonconforming fabrications be documented on an Inspection Report Form.

Paragraph 7.3.3 further requires such nonconforming items to be controlled in accordance with Section 10.0 of the ASME accepted QA manual.

Section 10.0 requires that all nonconforming items be immediately identified with an Inspection Report Form and placed in bond.

Contrary to the above requirements, all of the components for job numbers listed in Items B, C, and D'above were still in the manufacturing process 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> later and had not been identified with an inspection report or placed in bond.
