Dear Mr. Keppler:
On June 1, 1979, Mr. F. R. Hodges of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). During the placement of concrete in Pour No. 1CW-Ext.-2 the specified pour rate of 3 to 3 1/2 feet per hour was exceeded. A pour ra'.e of 10 feet in 70 minutes resulted in the breaking of a whaler on the lower portion of fermwork and possibly created a gap between the concrete centainment wall and containment liner, as iliscussed in PSI's interim report of June 1,1/181, it has been determined tl at a small portion of the containment liner will be removed to allow a closer examination of the concrete backing behind the liner. Previously plans had been for the major structural contractor to perform this examination.
J!th the recent return of the containment liner supplier, Chicago Bridge &
Iron,(CB&I), to the site, it has been decided that they will perform this examination. CB & I will require some time to prepare prior to undertaking this task and consequently this examination has not been performed yet.
This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A further report will be filed on or before November 1, 1981 .
