Appendix A NOTICE OF VIOLATION Dairyland Power Corporation Docket No. 50-409 As a result of the inspection conducted on June 15-18, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the follow-ing violations were identified:

Technical Specification 6.8.1 and 6.11 require adherence to radiation protection procedures.
LACBWR operating manual, Volume X, Health Physics Procedures, Section 6.5 requires the use of special work permits (SWPs) for any work where conditions are unknown and there exists a high potential for radio-logical h3zards. SWP 81-09 required the use of rubber g'.. oves during the decontaoination of a fuel cask.
Contrary to these requirements, rubber gloves were not worn by three workers involved in the cask decontamination. The failure to wer-the required gloves resulted in hand contamination of the workers.
This is a Severity Level V violation (Supplement IV).

Technical Specification 6.9.3(a) requires that a report of radioactive effluents to unrestricted areas be made to the NRC within 60 days after January 1 and July 1 of each year.
Contrary to the above, the required report for the six-month period ending December 31, 1980, was not submitted until March 31, 1981, a period in excess of 60 days after December 31, 1980.
This is a Severity Level VI violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance: -(1) corrective action taken and the res17ts achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Con-sideration may be given to extending your response time for good cause shown.
