APPENDIX A t'otice of Violation Portland General Electric Co.

Docket No. 50-344 121 S. W. Salmon Street Portland, Oregon 97204 As a result of the inspection conducted on June 1-30, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

i 1.

Technical Specification 6.8, " Procedures," states, in part, that: " Written procedures shall be established, implemented, and maintained covering the l

]

activities referenced below:

a.

The applicable procedures reconmended in Appendix "A" of Regulatory Guide 1.33, November, 1972...."

Regulatory Guide 1.33, November 3, 1972, Appendix "A," " Typical Procedures for Pressurized Water Reactors and Boiling Water Reactors," lists typical safety-related activities which should be covered by written procedures.

j Paragraph I, " Procedures for Performing Maintenance," states that,

" Maintenance which can affect the performance of safety-related equipment j

should be properly preplanned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the i

circumstances."

Section 6.8.2 states that, "Each procedure and administrative policy of 6.8.1 above, and changes thereto, shall be reviewed by the PRB and approved by the General Manager prior to implementation...."

l Contrary to this requirement, on June 4,1981, modification work requiring a freeze seal on the "C" loop RTD bypass linelto install a "T" fitting for t

the Reactor Vessel Level Indication System, under Request for Design Change, RDC 80-111,pias conducted with a freeze seal procedure not having unoer gone review by the PRB and the approval of the General Manager.

This is a severity level IV violation.

Section 2.C.7 of facility Operating License NPF-1 states:

...all j

activities to which a quality assurance program is applicable shall after l

the date of issue of the license, be conducted in accordance with the l

Quality Assurance Program for Operation." Quality Assurance Procedure QAP-9, " Control of Special Processes," requires, in part, that, "Each i

responsible supervisor shall ensure that personnel performing a special I

process under his direction are adequately trained, tested, and qualified, l

as necessary, prior to performing the special process activities."

l Contrary to this requirement, on June 4,1981, work requiring a freeze seal to be used was done by personnel who were not instructed in the application and cautions of using a freeze seal prior to commencing the work activity.

This is a severity level IV violation.
