Quick, Section Chief, Division of Resident and Reactor Project Inspection and other inspectors of this office on February 11-19, 1981, of activities authorized by NRC DPR-77 for the Sequoyah facility. Our preliminary findings were discussed with Mr. J. M. Ballentine, Pl Tnt Superintendent, and other TVA employees at the ~ conclusion of the inspection. Areas examinad during the inspection and our findings are discussed in the enclosed inspection report. W1 thia these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspectors. During'the inspection, it was found that certain activities under your license dppear to violate NRC requirements. These items and references to pertinent requirements are listed in the Notice of Violation enclosed herewith as Appendix A. Elements to be included in your response are delineated in Appendix A. The Sequoyah facility has been cited for a relative'y high number of vio'.ations j involving personnel error over the past year. The NRC'. Systematic Assessment of Licensee Performance appraisal conducted during 1980 also indicated a weakness in i the performance of the Sequoyah plart in this area. We have generally viewed past events and operator errors at the Sequoyah facility to be isolated in nature; however, the latest event, now leads us to conclude that significant improvements in your control of licensed activities is required. Certain corrective actions were set forth in our Confirmation of Action letter dated February 23, 1981. These matters and other corrective actions were discussed at a <ceeting with your staff in the Region II office on February 27, 1981. To further ensure that necessary corrective actions a e taken, your reply to this letter should specifically identify those short and long-term actioils you have taken ard l intend to tak9 to upgrade the personnel performance of the Sequoyah facility. Also, your reply should consider the applicability of your actions, if aporouriate, to other TVA facilities. I1 accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed report will be placed in the NRC's Public Document Room. If the report contains any information that you believe to be exempt from disclosure under 10 CFR 9.5(a)(4), it is necessary that you: (a) notify this of fice by telephone within ten days from the date of this letter of your intention to file a request for withholding; and (b) submit within twenty-five days from the date of this letter a written application to this office to withhold such informatta,. If your receipt oi this letter has been delayed such that less than seven days are available for your review, plaase notify thi: office promptb so that a new due date may be established. Consistent with section 2.790ic)(1), such application must be accompanied by an affidavit executed by tne cwner of the information which identifies the document or part thereof sought to be withheld, and a full statement of the reasons on the basis of which it is claimed that the information should be withheld from public l disclosure. This section further requires the statement to address with specificity the considerations listed in 10 CFR 2.790(b)(4). The information sought to be withheld shali be incorporated as far as possible into a separate part of the affidavit. If we do not hear frcn you in this regard within the specified periods noted above, the report will be placed in the Public Document Room.
