Mineral Area Osteopathic Hospital License No. 24-18040-01 ATTN:
Mr. C. J. LaRose Aduinistrator 1212 Weber Road Farmington, Missouri 63640 1
Gentlemen:
1 1
i This refers to the routine safety inspection conducted by Dr. L. W. Shatterly of this office on July 16, 1981, of activities authorized by NRC Byproduct Material License No. 24-18040-01 and to the v.iscussion.of our findings with you and selected members of your staff at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license 1
as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During the inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A l
written response, submitted under oath or affirmation is required. The noncompliance occurred before NRC adopted the Interim Enforcement Policy j
published in the Federal Register Notice (45 FR 66754) dated October 7, j
1980; therefore the items are classified in accordance with enforcement criteria then in effect. Had the over xposures described in Appendix A 1
occurred during this period, the new enforcement policy would have probably resulted in a civil penalty.
I This notice is sent to you pursuant to tb provisions of Section 2.201 of
{
the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.
1 Section 2.201 requires you to submit to this office within twenty days of i
your receipt of this notice a written statement or explanation in reply,
{
including for each item of noncompliance:
(1) corrective action taken and i
the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Be sure to address each of these items in your response.
