APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-353 Philadelphia, Pennsylvania 19101 License No. CPPR-107 As a result of the inspection conducted on June 2-5. 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,198C), the following violation was identified:
10 CFR 50, Appendix B, Criterion V, states, in part, that: " Activities affecting quality shall be... accomplished in accordance w'th these instructions, procedures, or drawings."
The Limerick PSAR. Appendix D, Paragraph 6.4, states, in part: "Bechtel Construction Department...is responsible for construction of the plant to approved engineering specifications...."
General Electric Specification 22A2724, Revision 3, Equipment Storage Requirements, Paragraph 4.1.2.l l.a. states, in part: " Internal cleanli-ness of equipment...shall be maintained by closing openings, where possible, by plugging, capping and/or sealing with tape.
Butt weld preps on valves
...shall have plywood fitted against the prep and tape sealed."
Contrary to the above, on June 3,1981. reactor coolant pressure boundary valve 2F023A was observed to have no plywood cover fitted against the weld prep and tape sealed.
This is a Severity Level VI Violation (Supplement II) applicable to CPPR-107.
Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Where good cause is shown consideration will be given to extending your response time.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
