APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Millstone Unit 1 Hartford, Connecticut 06101 License No. DPR-21 Docket No. 50-245 As a result of the inspection conducted on April 5 through May 16, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following viciation was identified:
Technical Specifications 3.1, 3.2.A and 3.2.B require a minimum number of two operable high drywell pressure instrument channels in each of two trip systems.
In the absence of the required number of operable instrument channels for both trip systems, an orderly shut down is to be initiated; all operable control rods inserted within four hours (Specification 3.1) arJ the reacter in cold shutdown within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> (Specification 3.2).
Contrary to the above, the reactor was made critical on April 17, 1981 at 2141 hours0.0248 days <br />0.595 hours <br />0.00354 weeks <br />8.146505e-4 months <br /> and again on April 18 at 0040 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br /> and operated until 2240 hours0.0259 days <br />0.622 hours <br />0.0037 weeks <br />8.5232e-4 months <br />, April 19 with one of two containment pressure instrument channels valved out of service in each of two trip systems. At that time those instruments were placed into service.
This is a Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company would be required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged violations; (2) the reasons for the item (s) of noncompliance if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
However, your corrective action was taken during this inspection period and has been described and verified in the attached inspection report. Additional corrective actions to prevent reoccurrence which you committed to implement are described in the attached Inspection Report and were documented in your letter to the NRC dated May 1, 1981.
