Purpose.

To promulgate guidance for assuring proper packaging and shipment. of radiographic sources f rom USS SIERRA and to ensure compliance with requaramen.ga set forth ir. references (a) chrough (e).

2.

Orkanization, The final responsibility for the Quality Assurance (QA) Program s.

for part. 71 requirements rests with USS SIERRA.

Design and fabrication shall not be conducted ender this QA program.

Radiography personnel shall perform the regulatory required inspections and tests in accordance with procedures. contained reference fe) and this instruc-tion..The Radiation Safety Officer shall ensure that these functions are pc.'..o rmed.

Quality Assurance Records Records of package (including references), procurement, inspections, a.

tests, operating logs, audit results, personni'l training and gatlifications and records of shipments will be maintained to accordance with reference (e) and enclosure (1) of this instruction.

These records will be maintained in accordance with references (b).

and (e) and will be identified and retrievable. A list of these records, and their storage location, will be maintained by the Radiation Safety Of ficer.

9.

Audits Schedules of internal audits, and their frequency (reference (e)), of a.

the Q.A. program will be conducted using a written check list with the results being reported through the Chain of Command.

The Radiation Safety Officer will maintain records of these audits which, will be evaluated by the Repair Officer and Radiation Safety Officer.

The RSO will. insure any deficient areas are corrected.

Yearly audits will be conducted in accordance with reference (d).

Members of the audit team shall have no responsibility in the activity being

10. Discussion.

All shipments c/ transfers of radioactive materials over public a.

areas, including shipments made with private or government vehicles, must.

comply with appropriate Federal, State and local. transportation requirements This instruction is.provided to ensure SlERRA complies with ref erences (a) through (e) with regards to shipment of radiographic sources.

Radiographic Isotopes are capable of inflicting serious bodily harm.if used improperly or without regard for safety, Organizational responsibilities for these Quality Assurance c.

functions and. audit guidance are set forth in references (d) and (e) and supplemented by reference (a),

11. Action.

The Radiographic Safety Officer.will ensure that all radio-graphic personnel are provided with.a copy of this instruction and its enclosure, and that all radiographic personnel comply with them.

Prepare requisition and invoice / shipping document (DD-1149).

Fill out parts 1, 2, 3, 4, 5, 6, 8 and 10 ae with a normal shipment.

b.

Parts (a) through (k) will consist of the following statements in sequence:

(1)

Shipping name.

(2)

Shipping class.

(3) Total quantity of material in curies as appropriate.

(4) Type of packing.

(5)

Name of radionuclide.

(6)

Description of physical and chemical form and serial number, if applicable.

(7)

Transport index.

(8)

Category of label applied.

(9). The statement " PACKAGE _ REQUIRES RADIOLOGICAL CONTROLS FOR UNPACKING.

RETURN RECEIPT REQUIRED."

(10)

The statement "This is to certify that the above named materials are properly classified, described, packaged, marked and labeled, and are in proper condition for transportation according to applicable regulations of the Department of Transportation."

(11) A statement of receipt.
