Use of Incorrect Size Terminal Lugs for Thermon Heat Trace System RTD Installations

Reference:

Southern California Edison 10 CFR 50.55(e) report of June 19, 1981, " Final Report on Unqualified Soldering Process in Thermon Heat Trace System Installation."

Dear Mr. Seyfrit:

On May 26, 1981, Southern California Edison San Onofre Units 2 & 3 infonned Mr. Engelken, U.S. Nuclear Regulatory Commission Region V of the subject deficiency. The final report was submitted on Jurs 19, 1981.

Evaluation by Bechtel revealed that incorrect size. terminal lugs were installed on Resistance Temperature Detector wire leads by both Bechtel and Thermon. Based on the evaluation, Bechtel concludes that the condition is reportable under 10 CFR Part 21. The Southern California Edison 10 CFR 50.55(e) report of June 19, 1981 contains all the information required by Regulation 10 CFR Part 21.

Thermon has been notified of their use of incorrect size terminal lugs and will be notified that Bechtel has reported the condition to the NRC.

Other Bechtel Divisions were advised of the defect by copies of appropriate reports and correspondence. Should it be determined that basic components with this defect are in use, have been supplied to or
