2-SI-73 FOUND OPEN 1.

EVENT DESCRIPTION

With the Unit at 100% power, valve 2-SI-73 (a containment isolation valve) was found in the open position while preparing for PT-18.1 (flushing of sensitized stainless steel piping). This is contrary to T.S.-3.8.2.A.1 and is reportable per T.S-6.6.2.b(2).

PROBABLE CONSEQUENCES:

Valve 2-51-73 is a containment isolation valve in the accumulator drain header. Although this valve was open, the two valves downstream of 2-SI-73 (2-SI-223 and 2-SI-184) were verified as being closed; therefore the health and safety of the public were not affected.

CAUSE

The cause of the valve having been lef t in the open position is unknown.

It was clcsed and verified closed after performing PT-18.1 dated 5-16-81.

On 6-14-81, 2-SI-73 was opened to perform OP-7.1 (recirculation of the accumulators with the EWST). The valve was again closed and verified as such on 6-14-81.

IEEDIATE CORRECTIVE ACITON:

The i=cediate corrective action was to close the valve once PT-18.1 was

completed.

SUBSEQUENT CORRECTIVE ACTION:

The subsequent corrective action was to verify that the valve was closed and chain locked in that position.

ACTION TO PREVENT RECURRENCE:

The present program for valve position v..rification requires that

~

two individuals verify the correct position " the valve following completion of testing. It is believed that this p ogram continues to be ef fective, and should preclude events of this type.
