Docket Nos. 50-361 and 50-362 San Onofre Nuclear Generating Station, Units 2 and 3 In a letter to your office dated June 1, 1981 we identified a condition which we considered potentially report-able in accordance with 10CFR50.55(e).
The condition concerns the use of brass or non-hardened steel washers by the supplier, ITT-Grinnell, in the assembly of certain mechanical shock sup-pressors in lieu of hardened steel washers as required by ASME Code.

I FINAL REPORT ON THE USE OF NONCONFORMING WASHERS ON ITI-GRINNELL SHOCK SUPPRESSORS San Onofre Nuclear Generating Station, Units 2 and 3 INTRODUCTION This report is submitted pursuant to 10CFR50.55(e)(3).
It describes a condition discovered during surveillance inspections conducted at the supplier's facility.
The condition involves failure to install ASME Code required hardened washers under the bolt heads on sizes 1/4 to 10 mechanical shock suppressors.
This report includes a description of the condition, an analysis of the safety implications, and a summary of corrective actions taken or to be taken.
By letter dated June 1, 1981, Edison confirmed noti-fication to the NRC of this potentially reportable condicion.
BACKGROUND On May 1, 1981, a Bechtel Quality Assurance Bulletin was issued which described a surveillance finding at the ITT-Grinnell Warren, Ohio, facility.
This finding indicated that, contrary to the requirements for hardened washers in ASME Section III, Subsec-tion NF, Paragraph NF-4724 - 1974 and 1977 Editions, the supplier had installed brass washers in sizes 1/4 and 1/2 mechanical shock suppressors.
In addition, the supplier had installed non-hardened steel washers in sizes 1 through 10 mechanical shock suppressors.
A brief survey of ITT-Grinne11 mechanica1 shock suppressors in-stalled at San Onofre Units 2 and 3 indicated non-code washers had been supplied.
DISCUSSION i
The following discussion is responsive to 10CFR50.55(e)(3).
Description of Deficiency The washers supplied with ITT-Grinnell mechanical shop suppressors sizes 1/4 through 10 are not in compliance with the requirements of ASME Section III, Subsection NF, Paragraph NF-4724 - 1974 and 1977 editions.
Analysis of Safety Implications An analysis of the safety implications has not been performed.
This condition is reported as a significant departure from the requirements of the ASME Code.

A letter dated June 9, 1981 hs3 been sent to the supplier, ITT-Grinnell, notifying them of this condition as potentially reportable.

All affected washers in ASME Code and safety related ITT-Grinnell mechanical shock suppressors installed in San Onofre Units 2 and 3 in sizes 1/4 through 10 will be re-placed with hardened steel washers.
