APPENDIX A NOTICE OF VIOLATION Metropolitan Edison Company Docket No. 50-320 Based on the results of an NRC inspection conducted on October 19 -

December 6,1980, it appears that certain of your activities were not conducted in full empliance with the conditions of.your NRC facility license No. DPR-73 as indicated below.

These are categorized as deficiencies.

A.

The Order for Modification of License, dated, July 20, 1979, as amended by the Order dated February 11, 1980, statas in part:

...Pending further amendment of the Facilit-, Operating License, the licensee shall maintain the facility ir. accordance witn the requirements set forth in Attachment 1..."

(proposed Technical Specifications, Appendix A to License No. DPR-73). The proposed Technical Specification 6.4.1 states:

"A retraining and replace-ment training program for the unit staff shall be maintained under the direction of the Supervisor-Station Training and shall meet or exceed the requirements and recommendations of Section 5.5 of ANSI N18.1-1971 and A'ppendix "A" of 10 CFR Part 55."

Contrary to the above, as of December 6, 1980, a retraining and replacement training program for the unit radiological controls department was maintained under the direction of the Supervisor-Radiological Training. The Supervisor-Radiological Training reported to the Manager-Radiological Controls.

B.

The Order for Modification of License, dated, July 20, 1979, as amended by the Order dated February 11, 1980, states in part:

...Pending further amendment of the Facility Operating License, the licensee shall maintain the tcility in accordance with the requirements set forth in Attachment 1..." (proposed Technical Specifications, Appendix A to License No. DPR-73). The proposed Technical Specification 6.8.1 states in part:," Written procedures shall be established, implemented, and maintained covering...the applicable procedures in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978." Three Mile Island Nuclear Station Administrative Procedure 1001, TMI Document Control, states in

' paragraph 3.8.5 that " Plant Operating Procedures shall be followed and signed off step-by-step...."

Contrary to the above requirements, (1) On October 3, 1980, Operating Procedure 2104-4.13, Revision 7, dated September 10, 1980, titled Transfer of Radioactive Resin Liners from EPICOR-I to On Site Staging Facility and from Staging Facility to Shipping Cask was not implemented as required in that an outdated procedure 2104-4.13, Revision 6, dated June 9, 1980, was used.
