y sw APPENDIX A NOTICE OF VIOLATION Gamma Corporation License No. 53-16847-01 649 - California Avenue Suite 102 Wahiawa, Hawaii 96786 As a result of the inspection conducted on May 7, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:
A.
10 CFR 20.401(a) states each licensee shall maintain records showing the radiation exposures of all individuals for whom personnel monitoring is required under 20.202. Such records shall be kept on form NRC-5 in accordance with the instructions contained on that form or on a clear and legible record containing all the information required by Form NRC-5. The doses en-tered on the forms or records shall be for time periods not exceeding one calendar quarter.
Contrary to the above, at the time of the inspection, personnel dosimetry records for two employees of Gamma Corporation were not available for the third quarter of 1980 (June through September). The thermoluminescence detectors (TLD's) for the third quarter of 1980, had not been sent in for processing.

License Condition 16 specifies that except as otherwise provided by the license, the licensee shall possess and use licensed material described in items 6, 7, and 8 in accordance with statements, representations, and pro-
.cedures contained in applications dated November 28, 1975, and October 22, 1976, and a letter dated January 19, 1976. The October 22, 1976 application specifies that the Victoreen Model 497 survey meter will be calibrated annually.
Contrary to the above requiremer', the Victoreen Model 497 was calibrated on November 18, 1977, and on Aprii z6,1979. The Victoreen Model 497 survey meter was utilized by the licensee on December 21, 1978, five weeks after it was due for recalibration.

.. Pursuant to the provisions of 10 CFR 2.291, the Gamma Corporation Wahiawa, Hawaii, is hereby required to submit to this office within thirty days of the date of this nntice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
