Perry Nuclear Power Plant Docket Nos. 50-440; 50-441 Interim Report on Crack in Concrete Wall / Slab

Dear Mr. Keppler:
This letter serves as the Interim Report pursuant to 10 CFR 50.55(e) concerning a major crack in a concrete wall / slab of the Fuel Handling Building. This condition was first reported by W. J. Kacer of The Cleveland Electric Illuminating Company to L. G. McGregor of your of fice on June 2, 1981 This report includes a description of the deficiency and the planned corrective actions.
Description of Deficiency The slab IB0-S10-599 is located on top of wall IB0-W37-597.
The crack is continuous through the wall and into the slab. The wall was placed on August 14, 1980; the slab was placed on December 10, 1980 This crack was identified as a result of subsequent routine quality control inspection and was visually evident through the surface of the wall.
Nonconformance Report NECC 841 was initiated and ongoing investigation has revealed the extent and location of the crack as illustrated on Attachments 1-5 of this report.

Mr. J ces G. Ksppler June 29, 1981 Planned Currective Action The concrete involved is in the process of being removed to expose the entire condition. After removal is completed, concrete will be placed in compliance with project specification and drawings.
At present, no additional exatples of this condition have been identified in other previously placed concrete and the planned date for submittal of this final report is October 1, 1981.
