GLEN ELLYN, ILLINOIS Go137 JUN 1 1981 Shores Nuclear and Ultrasound License No. 21-17223-01 Diagnostics Ram Gunabalan, M.D.

Suite 102 24025 Greater Mack Avenue St. Clair Shores, MI 48080 Gentlemen:

This refers to the routine safety inspection conducted by Ms. E. Matson and Mr. J. Mu'lauer of this office on May 12, 1981, of activities authorized by NRC Byproduct Material License No. 21-17223-01 and to the discussion of our findings with you at the conclusion of the inspection.

i The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license.

The inspection consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

During this inspection, certain of your activities appeared to be in non-l compliance with NRC requirements, as specified in enclosed Appendix A.

A written response, submitted under oath or affirmation, is required.

In addition to the item of noncompliance noted in Appendix A, we are concerned about the function of your Capintec dose calibrator. Records of dose calibrator daily checks indicate a variation from the expected values of approximately -7.5 percent for cesium-137 and +24 percent for barium-133.

In your application for licensure dated September 28, 1976, you stated any variation beyond 15 percent of the expected value is not acceptable for proper function of the instrument. Therefore, in you response to this letter, please describe what action you will take La assure prcper functioning of the dose calibrator.
