Dear Mr. O'Reilly:

On April 22, 1981, Mr. Virgil Brownlee ci the NRC Region II Office was notified of a potential substantial safety hazard involving the turbine driven emerg:.ncy feedwater pump lube oil cooler. An interim report was filed on May 22, 1963, stating that our review was not yet complete.

South Caroli,na' Electric & Gas Company (SCE&G) has now completed its review and has determined that this problem is a reportable substantial safety hazard under the provisions of IOCFR21. Bingham Willamette Company (prima contractor fo this equipment) has been notified of this report. Details are given on the attachment.

This is considered a "hal report.

Identification of Firm Supolying Component Terry Turbine Corporation through Bingham Willamette Company.

Nature of Defect, Substantial Safety Hazard Created and Evaluation _

The turbine driven Emergency Feedwater Pump lube oil cooler was not procured by Bingham Willamette Company from their subvendor to meet the proper design conditions. Although this equipment has operated properly during testing, without proper documentation its failure must be assumed. Failure of this heat exchanger would cause the loss of the turbine and the subsequent loss of this emergency feedwater train.

The specific cause of this problem will be addressed by Bingham Willamette Company.

Date Information of Defect Was Obtained April 22,1981.

Number and Location of Defect There is one lube oil cooler for the turbine driven Emergency Feedwater Pump located off the skid on elevation 412 of the Intermediate Building at column

Corrective Action The lube oil cooler is being repicced by a heat exchanger which meets all the design requirements. This effort is expected to be completed prior to fuel lond.

i Correccive action as related to the specific cause will be addressed by Bingham Willamette Company.
