APPENDIX A Notice of Violation Portland General Electric Co.

121 S. W. Salmon Street Portland, Oregon 97204 Docket No. 50-344 As a result of the inspection conducted on February 2-27, 1981, and in accordance with the Interim Enforcement Policy, 45 FR. 66754 (October 7,1980), the following violations were identified:

Section 2.C.(7) of Facility Operating License NPF-1 states:

...all activities to which a quality assurance program is applicable shall, after the date of issue of the license, be conducted in accordance with the Quality Assurance Program for Operation." Quality Assurance Procedure QAP-5, Procedures, Instructions and Drawings, requires, in part that, "... activities affecting quality shall be accomplished in accordance with current Plant Operating Manual procedures,..."

a.

Plant Operating Manual Procedure A0-3-18, System Pressure Tests, requires, in part, that a minimum of one manual valve be designated as a pressure relief path for over pressure protection.

Contrary to the above, a manual valve was not designated or used for over pressure protection during the system pressure test conducted on February 5, 1981.

This is a Severity Level V violation (Supplement 1).

b.

Plant Operating Manual Procedure 016-5, Replacement of Radioactive Filters, provides the procedural guidance necessary to remove a spent filter and replace it with a new filter.

i Contrary to the above, on February 20, 1981, when returning the reactor coolant filter to service, the drain valve was left open and resulted in a release of radioactive water and gas to the Auxiliary Building.

Tnis is a Severity Level V violation (Supplement 1).

Plant Operating Manual Procedure ONI 12, High Activity-Radiation Monitoring, requires, in part, that if a HIGH radiation alarm actuates in the Auxiliary I

Building ventilation exhaust channel PRM-2, then the Auxiliary Building supply and exhaust fans shall be stopped.

Contrary to the above, the Auxiliary Building supply and exhaust were not stopped when a HIGH alarm was actuated on PRM-2. This HIGH alarm was caused by an inadvertent release of reactor coolant to the Auxiliary Building on February 20, 1981.

This is a Severity Level V violation (Supplement 1).

2-Pursuant to the provisions of 10 CFR 2.201, Portland General Electric Company is hereby required to subnit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
