Your letter of March 30, 1981 forwarded results of an inspection conducted February 2-27, 1981 and three notices of violation.
Pursuant to the provisions of 10 CFR 2.201, attached is our reply to these notices of violation describing (1) the corrective steps which have been taken and the results achieved, (2) the corrective steps which will be taken to avoid further items of noncompliance. and (3) the date when full compliance will be achieved.

Attachment RESPONSE TO NOTICE OF VIOLATION a.
Notice of Violation Plant Operating Nbnual' Procedure AO-3-18, System Pressure Tests, requires, in part, that a minimum of one manual valve be designated as a pressure-
~
relief path for over pressure protection.
Contrary to the above, a manual valve was not designated or used for over pressure' protection during the system pressure test conducted on February 5, 1981.
This is a Severity Level V violation (Supplement 1).

Response
As required by Procedure A0-3-18, a manual pressure ~ control 1 valve was not included in the test procedure due to an oversight when the test instruc-tions were prepared. As the test had been completed prior to the audit finding, action to correct the specific problem noted was not possible.
To preclude recurrence, a training session will b'e conducted to' review.
the requirements of A0-3-18, System Pressure Tests. All plant engineering personnel who normally prepare pressure test instructions will receive-1 this training by April 30, 1981.

Notice of Violation PJant Operating Manual Procedure 01-6-5, Replacement of Radioactive Filters, provides the procedural guidance necessary to remove a spent filter and replace it with a new filter.
Contrary to the above, on February 20, 1981, when returning the reactor coolant filter to service, the drain valve was left open and resulted in a release of radioactive water and gas to the Auxiliary Building.
This is a Severity Level V violation (Supplement 1).
Response
OI-6-5 did not contain sufficient guidance, in that it never reminded the i
operator to shut the Orain valve.
It is not considered a procedural violation to coit an action not spelled out in the procedure. See item
- c. below for corrective action.

Page Two Contrary to the 'above, the Auxiliary Building supply and. exhaust were not stopped when e.HICH alarm was actuated.on PRM-2.-
This HIGH alarm was caused by an inadvertent release of-reactor coolant to the Auxiliary.
Building on Felruary 20, 1981.
This is a Severity Level V violation (Supplement'1)'.
Response
Notices of Violations b. and c. resulted from returning the.RCS filter to service and Icaving the drain valve open. When the filter was placed in service, Auxiliary Building activity. increased.
The valve' being lef t open caused the ABDT 3evel to increase, which overflowed backup into the
{
BIT room. Approximately 1.3 curies of noble gas was released. The Auxiliary Building fans were not stopped as required by ONI-12.. By the time the requirement to stop the fans was recognized (about 10 minutes),-
the alarm was able to be reset.
As a result of this event, the following actions have taken place:
- 1) A change to 01-6-5 has been submitted and reviewed by the PRB to add a step to close the drain valve before returning it to service.

- 2) This event and the requirements to follow procedures has.bcen:

reviewed with all operators. This was done by the Operations 3
Supervisor discussing it directly with the people involved. This was discussed with the Shift Supervisors and Assistant Shift Supervisors during a supervisory training class.
It was reviewed with all operators in a memorandum dated April 15, 1981.
