O Appendix A NOTICE OF VIOLATION The Ohmart Corporation License No. 34-00639-01 As a result of the inspection conducted on January 21-22, 1981, and tn accordance with the Interim Inforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
License Condition No. 16 requires that licensed material be possessed and usedia accordance with statements, representations and procedures contained in various dated applications and letters.
Letter dated June 28, 1978, states that monthly checks and semiannual complete calibrations will be performed and recorded for each survey meter.
Contrary to the above, no records of checks or calibrations were available for your Victoreen Model 440 (Serial No. 3062) survey meter.
This is a Severity Level 6 violation (Supplement 7).
