SUPPLEMENTAL TO FINAL REPORT' 1
i Cause The nature of the deficiency is such that an exact determination of
{
its cause is not possible. The deficiency was probably due to human error in the recording of heat numbers as the materials were logged

l Action to Prevent Recurrence
}
I t
The appropriate personnel have been reinstructed in the need for j
proper and accurate documentation of material heat numbers as i
specified in Receiving Inspection Storage and Preventative I
Maintenance Procedure C-501 and in addition, Quality Control i
Information Prc:edure, QCI-C-301.Il has been revised (as underlined) j i
to read:
Prior to unloading the material, verify that the heat number on t.he (**VA Requisition Form) 575 is acceptable for use per the latest list of acceotable heat numbers.
f I
This revision should prevent any material from entering the j
warehouse which is not on Phipps Bend's lists of acceptable material j
heat numbers (heat number log). Therefore, regardless of the cause
[
of an improper log entry (be it human error or otherwise), the error
}
would be detected by implementation of this procedure.
