Tube Turns had at this point shipped some p:oducts free this heat of steel identified as Lot Nu=ber W8806.
There were some fittings in the warehouse, in production and in our raw material yard.
All of this suspect material was pulled from production and tested by a metallurgical service cccpany hired by Amco.
At this point, a review was made to detemine if any of this potentially defective material was used for nuclear. application. It was determined that we had shipped two orders. One order was shipped to Hub Inc.-Piedmont-Hub.
This order was for 19-3" Standard Weight Lcog Radius 90* elbows SA234 WPB on their Order T-33060 ADD-Repl.
These elbows were shipped to Duke Power, Catawba plant.
The other order shipped to Chicago Tube and Iron, Chicago, Ill, on Order V14976.
Tnis order was for 22-21/2 x 2 Standard Weight Concentric Reducers SA23kWPB. These reducers were shipped to Baldwin Associates Clinton Power Station.
l Tube Turns notified Hub-Inc. and Chicago Tube & Iron on May 21,1980 by phone and in writing on June 10 to return subject fittings to Tube Turns.
Both Hub and Chicago Tube & Iron reported that these parts had been shipped to their custccers.
They i=nediately contacted Baldwin and Associates and l
Duke Power to return the fittings.
Tube Turns advised the NP.C originally on July 3,1980 that shipments of potentially defective fittings had been made to Nuclear Power Plants and that we had at that time not tested the material to determine if it was, in fact, defective.
The fittings which were at Duke Power were returned directly to Tube Etrns.
Tube Turns did not receive the 19 elbows until July 18, 1980.
The fittings which were at Baldwin and Associates Clinton Power. Station were returned to Chicago Tube
& Iron, who then returned them to Tube Turns on Sept. 2,1980.

'10/1/80 The fittings returned by Duke Power were tested on either S/9 or 8/12.
One of these pieces was rejected as beirg Armco J55 =aterial.
The cther pieces were accepted c.nd returned to stock, The potentially defective fittings returned from Clinton Power were tested on September 22, 1980, and all were found to be acceptable.
This letter now confirms that only one piece was defective and it is located at our Tube Turns Louisville plant.

Because of previous occurrences, Tube Turns has been instrumental in two = ills establishing alloy verifi-cation programs. Arsco did not have such a program, however, this was our first occurrence with their mixing materials.
Tube Turns has' instituted their own m y verifica-o tien p ogram.
This progra: began over a year ago l
and we have progressively been able to test mere material. Presently, we are verifying that each l
random length of pipe used on fitting crders is =ade frem the correct grade of material.
