. 4 APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket Nos. 50-334 and 50-412 Beaver Valley Power Station, Units 1 & 2 License Nos. OPR-66 and NPF-NPF-73 As' a' result of the inspection conducted on November 18 - December 31, 1989, and in accordance with the General Statement of Policy and Procedure for NRC Enforcement Actions,10 CFR Part 2, Appendix C, the following violation was identified:

Technical Specification 6.8 requires that written procedures be established, implemented, and maintained covering activities listed in Appendix A of Regulatory Guide 1.33, Revision 2, February, 1978.

Operating Manual Procedure OH 1.1.4W requires that when returning a train of Solid State Protection System to service, all trips be inhibited until the Low Pressurizer Pressure and Low Steam Line Pressure Safety Injection signals are blocked.

Contrary to the above, on December 13, 1989, Train A of the Solid State Protection System was returned to service without the Low Pressurizer Pressure and Low Steam Line Pressure Safety Injection signals blocked. This resulted in a Safety Injection actuation.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby required to submit to this office within thirty days of the date of the letter-which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
