m NOTICE OF VIOLATION i

Illinois Power Company Docket No. 50-461 Clinton Power Station License No. NPF-62 J,

As a. result of the inspection conducted from December 19, 1989 through-February 2,1990, and in accordance with the " General Policy and Procedures forNRCEnforcementActions,"10CFRPart2,AppendixC(1988),thefollowing violation was identified:

-10 CFR Part 50, Appendix B, Criterion V, requires that activities affecting quality shall be prescribed by documented instructions... and shall be accomplished in accordance with these instructions....

Clinton Administrative Procedure CPS No. 1019.02. " System Cleanliness,"

Paragraph 8.2.1 states, in part, "... when a safety-related system is opened for maintenance, the "as found" condition shall be documented on or attached to the work documents." Paragraph 9.1.1 states, in part, "Before final closure of a_ system after opening for maintenance or an operational evaluation, a visual inspection shall.be performed...." Paragraph 9.3 states, "For safety-related components the results of the inspection performed in Section 9.1 and 9.2 shall be documented and attached as part of the work package." The 1

Contrary.to the above:

a.

On January 18,_1990, the Division I,.16 cylinder diesel's heat exchanger was opened.for maintenance and the "as found" condition was not documented nor attached to the work documents.

b.

Or January 31, 1990, valve ISX005C in'the shutdown service water system was replaced during maintenance and the results of a visual inspection performed before final closure of the system were not documented nor attached to the work documents.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, you are required' to submit to this office within thirty days of the date of this-Notice-a written statement or explanation in reply, including for each violation: (1) corrective action

.taken and the results achieved; (2) corrective action to be taken to avoid further violatic,ns; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause
