There is increased interest in low-level waste issues in southern South Dakota because the Central States Compact has a site in northern Nebraska (within 5 miles of the South Dakota border) as its preferred low-level waste disposal facility.

Office of Nuclear Reactor Regulation items of Interest Week Ending January 26, 1990 TURKEY p0lWT. UNITS.3 AWD.4 the Atomic Safety and Licensing Board issued an Order for 16, 1990,(dismissal) of the remaining contention in an intervention ON January Sumnary Disposition proceeding.
In December 1988 an intervenor, Joette Lorien, challenged a license amendment which approved a revision of the plant's tech specs (TS) concerning pressure / temperature limits. The TS change made the P/T limits applicable to 20 full power years (instead of 10 years in the previous TS). The intervenor's concern related to reactor vessel embrittlement, and this general area of concern has been brought before the NRC on arevious occasions since 1980 by this same intervenor, once progressing as hig1 as the U.S. Supreme Court while challenging the pressurized thermal shock issue.
Significant staff resources were expended preparing for a hearing scheduled for February 27, 1990; the hearing is now cancelled.
The intervenor has 10 days to file a notice of appeal. The order will stand unless it is appealed or unless the Commission reverses it within 45 days.
ARKANSAS.WUCLEAR.0WE-(ANO) UNITS.I.& 2 On January 15, 1990, the Arkansas Power and Light Company (AP&L) announced several organizational changes. These changes are being implemented to continue the improvements begun with last summer's major reorganization which included the assignment of unit specific operation and maintenance responsibilities.
Highlights of this latest reorganization are the assignment of maintenance and engineering support staff to each plant specific maintenance organization; the assignment of systems engineers, performance engineers, and reactor engineers to each unit's systems engineering group; the establishment of a new engineering standards program department and a new nuclear industry support group.
In addition, the shift technical advisor positions are being transferred to nuclear operations as shift engineers reporting to unit specific shift supervisors.
Along with the organizational changes, AP&L also announced a planned relocation of all Little Rock based nuclear operations, engineering and support personnel to the ANO site by the end of the sunner. About 170 employees will be affected.

On January 19, 1990 the Atomic Safety and Licensing Appeal Board (ASLABP) issued ALAB-926 affirming the Licensing Board's February 2,1989 decision.
The February 2, 1989 decision authorized the disposal, by evaporation, of the 2.3 million gallons of accident generated water stored on-site.

As a result of NRC and NASA staff discussions held in September 1989, NASA's Director of the Division of Reliability, Maintainability, and Quality Assurance arranged an interagency meeting at a NASA contractor facility in Washington, D.C.

The Director, 01 and staff and the Chief, Vendor Inspection Inspection and staff attended the meeting. Each agency represented a brief description of its experiences with substandard / misrepresented parts and suppliers, its internal procedures and activities to disseminate the information among affected groups, and any external steps taken to inform other potentially affected federal agencies and/or industry organizations. The use of GIDEP as a vehicle for i
rapidly informing other groups was extensively discussed.
In the Fall of 1989, NRC became a member of GIDEP and now all NRC bulletins and information notices are made available to other member federal agencies through the GIDEP network. Although no final consensus was established, it was agreed that NASA Headquarters would convene a second meeting at the working group level in the near future with the aim of arriving at a mutually acceptable approach 1
for rapidly disseminating this type of information.
COMANCHE. PEAK.ORAT The Comanche Peak Operational Readiness Assessment Team began its two-week inspection on January 22, 1990. The inspection will provide an input and basis for an NRC determination of the applicant's readiness to load fuel and conduct power operations.

Similar reviews are scheduled for the other Category I facilities.

On January 23, 1990, representatives of BP Chemicals, Inc., met with NRC staff to discuss the planning of BP Chemicals for the continuation of the decontamination of equipment, grounds, and wastewater treatment ponds at their Lima, Ohio site.
BP Chemicals will submit its revised plans and schedules for the decontamination within a few months. The regulations on decommissioning funding assurance, due July 27, 1990, were also discussed.

On January 24,1990 Combustion Engineering, Inc., (CE) was issued an amendinent to Materials License No. SNM-33 authorizing enriched uranium processing in two l
new pelletizing lines. CE had initially been authorized on November'25, 1989 to move enriched material into the portion of the facility which housed the two new lines. CE expects to initiate operations immediately.

On January 18, 1990, LLWM staff met with DOE in Albuquerque, NM, to discuss the memorandumofunderstanding(MOU)fortheUraniumMillTailingsRemedialAction (UMTRA) project. The current MOV was signed in March 1987.
In September 1988, NRC and DOE reached agreement to streamline the NRC review process leaning to concurrences required by the Uranium Mill Tailings Radiation Control Act (UMTRCA).
During the meeting, discussions were held relative to changes to the MOU to incorporate the streamlining agreements and other changes in the program since the last MOV was signed. DOE agreed to prepare a marked-up version of the MOV for NRC review by February 2,1990.

The NRC Special Comittee to Review the Severe Accident Risk Report met on January 18 and 19,1990 in Bethesda. The Committee received additional
~
technical briefings by NRC, Sandia and Brookhaven National Labcratory personnel on selected topics relating to the NUREG-1150 report.
At the conclusion of these presentations, the Comittee agreed on an outline and content for the Committee's final report and made assignments for draf ting sections of that report. The next meeting of the Comittee will be March 8-9 at which time the Comittee will review and edit the working drafts prepared by the members.
The Comittee discussed some generalized observations regarding the 1150 report.
In general, the Comittee felt that the report was a highly valuable i
study, it illuminated the present state of PRA and includes new and powerful methods, notably the expert elicitation process and the latin hypercube statistical sampling method. The Comittee noted a number of areas which they believed were weak, mainly in the area of human reliability and sensitivity 4
analyses.

On January 23, in response to a notification by the Atomic Control Board of Canada regarding an event that occurred during an on-line refueling operation at the Bruce Generating Station-A, the NRC Incident Response Center notified

NRC management and other federal agencies and then monitored the progress of the event until its conclusion on January 24.
]
On January 24 the AEOD Director, Regional Administrator, Region V, the Associate Director for Reactor Projects, NRR, Palo Verde DET Manager and Deputy, and other NRC staff met at the NRC's Region V office with the President and Chief Executive Officer, Arizona Public Service Company, the Executive Vice President, Nuclear, and several of the top managers responsible for the facility. At this meeting the NRC provided the preifminary results of the Diagnostic Evaluation.
On January 19, TTC staff met with personnel from NRR/R$1B at the TTC to discuss the course outline, course instructors and course materials for a 3-day course for Electrical Distribution System Functional Inspection (EDSFI) inspectors.
i The TTC will support NRR through its Specialized Technical Training Task Order Contract with tie first of two courses planned for July / August 1990.
On January 22-25, the TTC staff conducted special simulator training for RES personnel with responsibilities in the severe accident management area. This training involved demonstrations of significant events and the associated mitigation efforts through the use of symptom-based emergency operating procedures (EOPs). The training was conducted on the NRC controlled simulators at the TTC which model the General Electric and Westinghouse reactor vendor designs.
On January 22-26, the first Internal Dosimetry /Whole Body Counting Course (H-312) was conducted 6t the TTC. Attendees included personnel from each of the regions and the states of California and North Dakota. The course was conducted by Roy F. Weston, Inc., under the TTC radiation protection task order contract.
Course instructors included Dr. John Poston, a leading expert on internal dosimetry. The course also included a field trip to the Tennessee Valley Authority Power Operations Training Center whole body counting facilities for equipment demonstrations.

Implementation.of NUDOCS/AD j
IRM has initiated actions in response to NUDOCS users expressing concern about slowness of response time in NUDOCS/AD. The NUDOCS contractor is leasing, on an interim basis, a Data General MV/20000 minicomputer. The computer was delivered on Friday, January 19 and was operating by Tuesday, January 23. The replacement computer has greater processing capacity and has improved the respnse time by a factor of four. Further system hardware improvements are planned for the week ending January 26 that will again increase the processing capacity and improve response time.
The leasing of the MV/20000 is a temporary measure, until NRC can complete the acquisition of the Data General MV/40000 that NUDOCS/AD was designed to operate in.

Decomissioning A Decomissioning File has been created, entitled, " License Terminated /Decomissioned," to be included as part of each official case file. Documents to be filed in this category include all incoming and outgoing correspondence and internal memoranda concerning license termination, cancelled applications, withdrawn applications and decomissioning information.

Event. Notification.Searth..acd. Reporting. System The parallel processing period ended successfully and the Data General minicomputer has been removed from the NRC Operations Center. AEOD, NRR, and the Regions are now actively using SINET to retrieve 50.72 information.
4.
FIS 2000. Network The Telecommunications Branch accepted the FTS 2000 network after successful transition of White Flint and Nicholson Lane Buildings on January 24, 1990.

i Office of Enforcement Items of Interest Week Ending January 26,1990 Significant Enforcement Actions A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $75,000 was issued on January 22, 1990 to Arkansas Power and Light Company (ANO Unit 1). The action was based on the miswiring of the control circMtry l
for two of the three service water pumps for Unit 1.
Under certain conditions following an automatic reactor trip, this problem would have prevented the automatic restart of the pumps. The escalation and mitigation factors of the Enforcenent Policy were considered and in this case the civil penalty was increased by 50%. The licensee's corrective actions warranted 50% mitigation but this was offset by 50% escalation for prior poor performance and escalation for an additional 50% for NRC identification.

On January 17 1990 Monongahela Valley Hospital informed Region I of a malfunctionofarem,oteafterloadingbrachytherapyirradiator,adevice that uses sealed radioactive sources to treat tumors. The device, a Curietron 2E1000, was manufactured by CIS-U.S., a French-owned company.
The malfunction resulted in the disconnection of the tube used to transfer thesources(inthisinstance, cesium-137)fromtheshieldedstorageunit to the patient while the patient was undergoing treatment of a uterine 1
I tumor. The disconnect resulted in one of the cesium-137 sources being located for an undetermined time near the upper part of the patient's leg, rather than in the patient. The licensee has estimated a range of j
potential unintended dose to the patient's leg from 23 rads to 23 700 rads,dependingonthelengthoftheexposureandtheproximityofthe J
source to the patient's leg. However, three physical examinations of the patient have indicated no visible signs of radiation damage to date.
Additional future examinations of the patient will be performed. The licensee subsequently reported two additional equipment problems with this device on January 18, but neither incident resulted in further unintended radiation exposures. The device has been removed from service pending evaluation by the manufacturer and the NRC. Region I dispatched two inspectors to the facility and notified the only other user of this j
cevice in the U.S. of the malfunctions at Monongahela Valley Hospital.
Region I is evaluating potential generic implications of this incident.

Medical. Canter.Wospital.of Varmont On January 17, 1990 a representative of the licensee informed Region I of the inadvertent activation of an AECL Theratron 780 teletherapy device containing a radioactive sealed source of cobalt-60 of approximately 3600 curies. The teletherapy source is used in the treatment of cancer patients by external beam. A patient was positioned under the teletherapy device for treatment and the technologist was setting the timer. The licensee stated that it appears that an electrical static discharge caused f
the treatment time value set on the timer to disappear. At the same time, l
the telethera]y source was achated to the unshielded position. The tech-nologist quiccly operated the emergency stop controls to prevent further exposure to the patient. The calculated dose was included in the l
patient's subsequently administered treatment. No unwarranted exposure to i
the aatient occurred. The licensee reported that similar static i
i disc 1arges had been noted over the past two years, but that this was the first time the source was actuated to the unshielded position.
l-NRC Region dispatched two inspectors to review the event with the licensee L

Seabrook On Wednesday,1/24/90, a subpoena was served by the NRC on Mr. Fred Anderson, Jr., President, Ideas and Information, Inc., Exeter, New Hampshire. The subpoena calls for Mr. Anderson to provide, by 1/30/90, tapes or copies of tapes he made of Seabrook control room radio transmissions to plant personnel throughout 1989 and in January 1990.
l About 200 voice activated recording tapes of 90 minutes duration each are reportedly involved. Mr. Anderson has stated that he will comply with the l
su)poena.

Pilgrim. Salt Service.WaterJuss CoJuan.Ianure On January 11 a deep draf t service water pump column assembly fractured when one end of it fell six to twelve inches into a concrete floor as it was being transported horizontally between buildings. The 43 foot long column fractured 360 degrees around the circumference in a brittle manner.
Subsequent testing revealed severely degraded tensile strengths and r
ductility in areas of the column sections. Boston Edison Company is performing additional analyses and materials testing to address operability of the four pumps in service. Preliminary evaluation indicate that the poor material properties are the result of problems in the original casting process. Boston Edison Company has issued an information report over INPO's Nuclear Network and the NRC staff is preparing a generic communication on the subject.

'The exercise was observed by. a team lead by Region Il that included Regional, henoquarters, and contractor team members. The exercise was successful, with no exercise weaknesses identified. There was local media interest and news coverage of the exercise and the results.

AmerJean.Jtadiolabeled. Chemical American Radiolabeled Chemicals is a firm located in St. Louis, Missouri, that manufactures a broad variety of research chemicals tagged with radionuclides. On January 11, 1990, an Order Suspending Licensed Activities was issued to the licensee based on radiological safety issues and apparent willful violations. The licensee requested a management meeting with Region III staff to discuss conditions necessary to return the arogram to operation. This meeting was to be held on January 25, 1990, but 1as been postponed pending the completion of Region III's inspection report and discussion of pending NRC enforcement action with other federal agencies.

Zion. Nuclear. Power Station On January 16, 1990, the 1A turbine-driven auxiliary feedwater pump (AFWP) was stopped during testing after running and cavitating for one minute. It was determined that the pump's discharge and recirculation valves were shut due to an inadequate procedure. Approximately one minute later the 1C motor-driven AFWP tripped due to low suction pressure caused by steam from the sc M tating 1A pump that entered the common suction header. This placed the unit in an 8-hour LCO.- The unit was in Mode 7,
. physics testing, at the time. The IC AFWP was vented, tested and returned to service on January 17, 1990. Unit I went into a 7-day LCO because the 1A AFWP was out of service. The 1A AFWP was taken apart and the seals replaced. The pump was returned to service January 23rd. During the return to service procedure a valve lineup error was made which was discovered later that day during testing of the IB motor-driven AFWP.
This error would have resulted in the IB pump delivering auxiliary feedwater to only two steam generators.

Davis Besse. Unit.1
-On January 18, 1990, enforcement discretion was granted to Toledo Edison Company to operate the unit with one parameter (flux-delta flux-flow) of the Channel 2 Reactor. Protection System RPS) not fully operable and not tripped. Technical Specifications ( S) 3.3.1.1 require an inoperable channel to be placed in the tripped condition, thus placing the plant RPS logic in a one-out-of-three logic. The relief was granted to avoid subjecting the plant to an unnecessary plant trip should any one of the eight parameters in any of the other three RPS channels experience a spurious trip.
In addition, it was also recognized that while the flux-delta-flux-flow parameter was inoperable, it could still perform its intended function but approximately 0.07% full reactor power above the required trip setpoint limit. The licensee has stated that should any other component in the flux-delta flux-flow logic in Channels 1, 3 or 4 degrade beyond its TS limit, they would follow the appropriate actions of TS 3.0.3.
The relief is to remain in effect until the commencement of the sixth refueling outage scheduled for February 1, 1990.

Senior.Nanagement.Neeting Region V hosted the Senior Management meeting from January 21-23, 1990.
Chairman C6rr, the EDO, Office Directors and Regional Administrators met in the regional office for two and a half days. The primary focus of the meeting was discussion of problem plants.
2.
Rancho.Seco. Nuclear Generating. Stat. ion The unit completed defueling in early December 1989. Major activities now in progress-include the placement of systems and equipment in a layup condition, continuing efforts to locate a small leak (1 gpm maximum)_ in the spent fuel pool, preparation of a submittal for request for 6
possession only license, and preparation for submittal of their decommissioning plan. The licensee has submitted to the NRC a number of license amendment requests and exemption requests in support of the closure of the facility. The submittals include:
a license amendment which would prohibit movement of fuel into the Reactor Building without NRC approval; a license amendment which adds specific defueled technical specifications; an exemption request for relief from the requirement to have a site specific simulator.(the licensee has stopped all work on their simulatorproject);
a request for significant relief in the areas of their Physical Security Plan, Emergency Plan, and Training Program. The licensee intends to allow INP0 accreditation of their training programs to lapse; and

a request for relief from the requirements of 10 CFR 50, Appendix J, l
containment Leak Rate Testing.
l The above items are under review by NRR. The staffing at the site stands L
at approximately 611, including the contract security force, down from L
-1650 when the unit was operating.
l The SMUD Board of Directors are considering whether the SMUD staff should l
close the plant or whether the work should be contracted out. A decision is expected soon on this issue. ~According to local press reports, the SMUD Board may also consider replacing the SMUD General Manager, Dave Boggs. This report follows the recent election of Ed Smeloff as Board President. Mr. Smeloff has been described by the media as " Anti Rancho Seco" and was a driving force in the election which resulted in tho L

plant shutdown. The media has also reported that the SMUD Board feels that the NRC is inappropriately requiring activities to be performed at the site, considering the status of the plant. Representative Vic Fazio (D-West Sacramento) visited the site on January 18, 1990. According to j
media reports, Representative Fazio stated his intentions to sensitize the NRC to the Rancho Seco situation. The NRC staff has previously reminded 1
the licensee that all applicable rules and regulations remain in effect until their license is amended.

Chairman Carr visited the Trojan Nuclear Power Plant on January 23, 1990. The visit included discussion's with Corporate and Plant management officials regarding plant performance and a tour of the j
facility.

On January 24, 1990, Chairman Carr visited Children's Hospital:of San.
Francisco to discuss the proposed change to 10 CFR Part 35 to include a basic Quality Assurance (QA) Program. A discussion was held with
-Dr. Hirsch Handmaker and the Chairman toured the Hospital facilities.
Dr. Handmaker was a major contributor to the QA manual developed by the Joint Commission on the Accreditation of Hospitals. Ed Bailey from the' State of California and Bob Pate from NRC Region V were at the meeting.

Arizona Nuclear. Power. Project On January 23, 1990, the Palo Verde AEOD Diagnostics Team held its exit meeting with APS and Region V at the Region V office. AE0D stated that the team inspection report would be issued by mid-February. AEOD l
