This refers to the inspection conducted by Mr. J. Russell of this office on-

December 18 through 22, 1989, of activities autt.orized by NRC License No.

NPF-1 and to the discussion of our findings held by Mr. Russell with Mr. Yundt

and other members of your staff at the conclusion of the inspection. .

Areas examined during this ins)ection are described in the enclosed inspection

.

report.

Within these areas, tie inspection consisted of selective

examinations of arocedures and representative records, interviews with

. personnel, and caservations by the inspector.

'

Based on the results of this inspection, it appears that some of your-

activities were not conducted in full compliance with NRC recuirements as set

forth in the Notice of Violation, enclosed herewith as Appencix A, and the

- Notice of Deviation, enclosed herewith as Appendix B.

'

Your response to these Notices is to be submitted in accordance with the

!

- provisions of 10 CFR 2.201 as stated in the~ Appendices.

We note that item A of Appendix A is a repeat of violation B. 3. in our letter

to you dated March 15, 1989.

Both violations are very /NRC management meeting

similar in nature and

have to do with work control practices.

During the PGE

on January 12, 1990, you presented your enhanced nuclear division improvement

plan and stressed steps being taken to improve maintenance.

In your reply to.

.

the enclosed Notice of Violation, we would be interested in seeing your

,

analysis of the causative factors for these similar violations and.an

explanation of how your improvement plan should have or will preclude

,

recurrence.

In addition to the repetitive aspects of item A, enclosed, we have~ concern

regarding the quality of your own review of matters related to the monitoring

i i

of containment depressurizations via the hydrogen vent system.

enclosed inspection report describes your use of questionable monitoring data

to discount s)ecific statements of a worker which clearly imply that the wrong

train of the lydrogen vent system may have been monitored during the mid-July.
