I APPENDIX B Masoneilan North American Operations Avon, Massachusetts NOTICE OF HONCONFORMANCE As a result of the inspection conducted on December 12-16, 1988 at the Masoneilan-Dresser (MD) facility in Avon, Massachusetts, it appears _that cer-tain of your activities were not conducted in accordance with NRC requirements.
Criterion IV, " Procurement Document Control," of Appendix B to 10 CFR Part 50 states, in part:
" Measures shall be established to assure that applicable regulatory requirements...and other requirements which are necessary to assure-adequate quality are suitably included or referenced in the docunents for pro-curement of materials...to the extent necessary, procurement documents shall require contractors / subcontractors to provide a quality. assurance program con-sistent with the pertinent provisions of this appendix."
Criterion Vll, " Control of Purchased Materihl, Equipment and Services," of Appendix B to 10 CFR Part 50 states,-in part:
" Measures shall be established to assure that purchased material... conform to the procurement documents...
measures shall include provisions, as appropriate, for source evaluation and selection, objective evidence of quality furnished by the contractor... exam-ination of products upon delivery... documentary evidence...such as codes, standards, or specifications met by the purchased material...."
Section 4, " Nuclear Procurement Control," of the MD Huclear Quality Assurance Manual, Revision J, dated June 1,1987, states, in p(PMC) specification is art:
"... Requirements are placed on the...P0.... A Purchased Material Control pro-vided tre production planning...to further define the code contract and quality requirements. The PMC becomes an attachment and part of the P0...PMCs shall require certification from the vendor that material was manufactured or supplied

in accordance with a quality program approved by Nasoneilan or ASME. The buyer vamines each requisition then initiates a Nuclear Purchase Order..He negoti-i ates the purchase with a vendor selected from the Qualified Nuclear Vendor Listing...."
Sections QS2.2, QS4.1, and 054.2, " Order Management" and " Procurement Control,"-

of the MD Commercial-Grade Quality Assurance Manual, Revision G, dated July 1, 1988 states, in part:
...when the nature of the order warrants project i
handling.the contract administrator shall coordinate a Project Order Review with engineering and quality assurance....The project order review members verify that the customer order requirements for materials, examinations, test-ing, inspections, documentation, and instructions are adequately identified in the Work Order....The Purchase Requisition is initiated by Production Con-i trol Form... Work Order, and/or the Material Pre-Order Form....The buyer shall examine each requisition and initiate a purchase order as a-formal procurement document...."

Contrary to the above, it was identified that MD failed to adequately execute its QA program requirenents as specified below (88-01-02):-
1.
MD failed to ensure that adequate cuality requirements were included or referenced on its P0 documents to the subcontractors listed below even though the requirenents of Appendix B to 10 CFR Part 50 and AftSI N45.2 were imposed on MD by licensee P0s; and 2.
MD failed to ensure that its measures to control the purchases listed below included adequate provisions to'either use an MD approved supplier, have objective evidence of the quality of the hardware provided, or per-form receipt inspection of the hardware.
