Dear Mr. Sanford:

This letter addresses the inspection of your f acility at Avon,- Massachusetts.

led by Mr. J. J. Petrosino of this office on December 12-16, 1988, and the discussion of his findings with you and otner members of your staff'at the conclusion of the inspection.

Release of this report was delayed during'NRC's ongoing review of nonconforming and. substandard vendor products.

7 The inspection was conducted as a result of. an October 21, 1988 10 CFR Part 21 report by Consumers Power Company regarding safety-related Masoneilan-Dresser Industries (MD). valve internal replacement parts that.were found to be manu-factured by a secondary manufacturer that-is not an authorized MD facility.

Areas examined during the NRC inspection and our findings are discussed in the enclosed report.

This inspection consisted of an examination of procedures and reprasentative records, interviews with personnel, and observations by the inspector.

During this inspection it was found that-the implementation of your quality assurance (QA) program failed to meet certain NRC requirements.--The most~

significant problems identified concern MD's' failure to establish adequate-measures to control material-procurement and traceability for valve internal-parts that are identified for use in safety-related systems and the failure to pass down quality control requirements to subtier vendors for safety-related orders.

It was also observed that MD does'not appear.to implement its 10 CFR Part 50, Appendix B program requirements for-parts that are exempt or outside.

the scope of Section III of the ASME> Boiler and Pressure Vessel Code. This approach appears to be used consistently whether or not a licensee imposes Appendix B or ANSI N45.2 quality requirements or states that;the order is.

nuclear safety-related. The specific findings and references to the pertinent' requirements are identified in the enclosures to this letter.

The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are. required to sub -

riit to this office within 30 days from the date of this letter a written state-ment containing:

(1) a description.of steps that have been or will.be taken to correct these items; (2) a description of steps that have been or will be-taken to prevent recurrence and (3) the dates your corrective actions and preventive measures were er;will be completed.

You are also requested to p'

submit a similar written statement for the item which appears.in the enclosed-ff

Notice of Nonconformance. We will consider extending the response time if you can show good cause for us to do 50.

The response requested by this letter is not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 19B0, PL 96 511.
