This ~1etter transmits the results of the routine inspection conducted by
Messrs. W. Schmidt and R. Plasse of this office on October 8,1989 through
+
December 2,:1989, at the James A. FitzPatrick Nuclear Power' Plant, Scriba, New
York. Mr. Plasse- discussed the inspection findings with you and other members
"
of your staff at.the conclusion of the inspection.
The inspector identified
no violations. You need not reply to this letter.
The causes for several events discussed it, the report concern us.
It appears
that personnel _ inattention to detail during surveillance testing caused the
reactor scram on November 5,1989, and the reactor core isolation cooling sys-
tem isolation on November 29, 1989. Further, we believe that actions taken by
,
you. to resolve previous difficulties should have prevented the poor high
,
pressure coolant . injection system tagout during system maintenance and the
'
difficulty experienced with the automatic depressurization system Technical
Specifications. We look forward to increased management attention and improved
performance in these areas.
