i APPENDIX A NOTICE OF VIOLATION Louisiana Power & Light Company Docket: 50-382 Waterford Steam Electric Station, Unit 3 Operating License: NPF-38 During an NRC inspection conducted December 1-31, 1989, a violation of NRC requirements was identified. The violation involved failure to follow independent verification procedures.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is listed below:
Technical Specification (TS) 6.8.1.a requires, in part, that written procedures shal's be implemented as recommended in Appendix A of Regulatory Guide 1.33 Revision 2, February 1978, which includes administrative procedures for safety-related activities.
Administrative Procedure MD-1-025. Revision 0, " Independent Verification Program " Section 5.2, requires an independent verifier to watch the other person perform the task designated for verification in the applicable procedure and verify that the task is completed as prescribed.
WorkAuthorization(WA) 01051422 implemented the above program by mquiring independent verification of each step in the procedure for charging nitrogen into the main steam isolation valve operator cylinders.
Contrary to the above, on December 26, 1989, a maintenance technician charged the A main steam isolation valve operator with nitrogen in accordance with WA 01051422, without the independent verification being perfomed.
This is a Severity Level IV violation. (Supplement I)(382/8941-02)
Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light Company is hereby required to subiait a written statement or explanation to this office, within 30 days of the date of the letter transmitting this Notice. This reply should include for each violation:
(1) the reason for the violation if admitted,(2)thecorrectivestepswhichhavebeentakenandtheresults achieved. (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
