Because there may have been some changes in your operation sin;e that time, it will be necessary for you to submit the following information for renewal of your license:
1.
The information outlined in Item a. of Section 70.22, 10 CFR Part 70 (enclosed).

The name of the person who will be responsible for.co-ordinating the use of your Pu-Be source and for insuring compliance with the terms and conditions of your NRC License.
3.
Your means of preventing unauthorized use or removal of the_ source from your use and storage area.
4.
A description of your procedures for leak testing the source including analysis of the leak test samples.
5.
If the source will be handled by students, a_ copy of the safety precautions used in the handling of the source.
6.
Procedures for handling and storing targets activated by. neutron exposure.

Include the names and telephone numbers of individuals within the college who should be notified and who will, in turn, notify the NRC and if applicable, the local police and State personnel. Please also inform us of your means for insuring that appropriate personnel are aware of the procedures to be followed.
