Enclosed is one copy each of the revised NRC Forms 396 and 398 (Enclosures 1 and 2).
The changes to NRC Form 396, Certification of Medical Examination by Facility Licensee, and NRC Form 398, Personal Qualification Statement - Licensee, are detailed in Enclosures 3 and 4 respectively. These revisions were made to reflect the changes of the operator licensing process.
Please ensure that all applications for licenses are submitted to the Region III Office on these revised forms.
Submittals using the previous forms will not be accepted after February 1, 1990.
