APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No.
50-245 Millstone Nuclear Power Station Unit 1 License No. DPR-21 During a routine resident inspection from September 6 through October 20, 1989, the following violation was identified.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C, (Enforcement Policy), the violation is listed below.
Plant. surveillance technical specification 3.2, Protective Instrumenta-tion, Table 4.2.1, requires that a monthly coannel instrument functional test be performed for the " Isolation Condenser Isolation System Steam Line l
High Flow" and " Isolation Condenser Isolation System Condensate Line High l
Flow" systems.
Surveillance procedure SP-412L, Isolation Condenser Isolation Instrument Functional and Calibration Test, Revision 9, l
Change 1, provides the specific functional test procedure.
l Contrary to the above, on October ', 1989, the licensee determined that the above tests had not been performed at the required monthly interval by i
September 25, 1989 pursuant to technical specification 3.2 and procedure SP-412L.
Corrective actions taken for a recent licensee-identified violation, that
-also involved a technical specification surveillance not performed within 1
the prescribed time interval, were ineffective to prevent recurrence.

j Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is 4
hereby required to submit to this office, within thirty days of the date of the i
letter which transmitted this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
