This refers to the special inspection conducted by Mr. C. W. Caldwell of this

office on October 7 through November 22, 1989 of activities authorized by NRC

License No. DPR-13 and to the discussion of our findings held by Mr. Caldwell

with you and other members of the Southern California Edison staff at the

conclusion of the inspection.

Areas examined during this inspection are described in the enclosed inspection

report. Within these areas, the inspection consisted of selective examina-

'

tions of procedures and representative records, interviews with personnel, and

'

observations by the inspector.

Based on the results of this inspection, it appears that certain of your

activities were not conducted in full compliance with NRC requirements, as set

forth in the Notice of Violation, enclosed herewith as Appendix A.

ticular, as discussed with you during our management meeting on November 29,

'

l

1989, Southern California Edison Company (SCE) did not take adequate correc-

tive actions to preclude failure of the Automatic Switch Company (ASCO)

solenoid operator for charging isolation valve CV-304.

In addition, SCE did

l

not consider hot leg recirculation to be a Technical Specification (TS)

system. These findings emphasize the need for SCE to critically evaluate

I

programs to control problem equipment and to implement TS requirements.

Your response to the enclosed Notice is to be submitted in accordance with the

provisions of 10 CFR 2.201, as stated in Appendix A. Notice of Violation.

In

addition to addressing the specific violations cited, your response should

also address actions taken or to be taken to ensure proper attention in the

future to weaknesses in corrective action programs.

In accordance with 10 CFR 2.790(a), a copy of this letter, the enclosure, and

your response will be placed in the NRC Public Document Room.

The response directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
