Inspection on June 2-3, 1981 (Report No. 50-454/61-06)

Areas Inspected: Routine announced inspection to review preoperational test

procedures; previous open items relating to the preoperational test program;

and preoperational testing quality assurance. The inspection involved 39

inspector-hours onsite by three NRC inspectors.

Results: No items of noncompliance or deviations were identified.

Licensee Action on Previous Inspection Findings

(Closed) Open Item (454/80-23-01): The licensee reviewed a listing

a.

of recent precperational test problems encountered at other plants

in order to ensure that administrative controls were in place to

minimize the occurance of similar problems at Byron.

The review,

subsequent discussion and actions taken was documented in a letter

dated April 1, 1981 from R. C. Ward to R. E. Querio. The inspector

reviewed the letter and determined the licensee's actions were in

accordance with their commitment and were acceptable.

b.

(Closed) Open Item (454/80-23-02): This item dealt with a lack of

instructi;ns for valve and switch lineups. The licensee developed

instructions for valve and switch lineups and incorporated them into

the training program for all individuals who may be performing lineups.

The inspector reviewed this program and considers it acceptable.

c.

(Closed) Open Item (454/80-23-03): This item was administrative in

nature and composed of three parts:

(1) The inspector reviewed the licensee's station procedure for

control of obsolete or incomplete test procedure copies and

determined the guidance it contained to be acceptable.

(2) The inspector resiewed the licensee's station procedure for

test procedure issue, distribution and control of test records

and determined it to be acceptable.

(3) The inspector reviewed the proposed wording for a change to

the Startup Manual to ensure that all (major and minor) test

procedure changes get incorporated into all " approved for test"

copies of the procedure and determined it to be acceptable.

(Closed) Open Item (454/80-23-04): -The inspector reviewed Rev. O

of BAP 300-5 dated February 1981 on jumpers and lifted leads and

determined that the licensee had incorporated provisions for inde-

pendent verification on. installation of jumpers and utilized a

system of return of. individualized tags to provide independent

verification of removal. The licensee decided not to develop a

-

specific procedure to control mechanical items such as removal

of temporary strainers but to treat them as turnover deficiencies

instead. The inspector determined the above items to be acceptable.

(0 pen) Qpen Item (454/80-23-05): This item dealt with areas of the

Startup Guidelines document which the inspector determined would be

more appropriately' contained in the Startup Manual.

Since Revision

2 to the Startup Manual eliminated the guidelines document altogether,

the scope of some of the comments has changed.

The inspector reviewed

the comments with the licensee. This remains an open item pending

incorporation of comments and further discussion with the licensee.

(Closed) Open Itsm (454/80-23-06): This item dealt with changes to

the Startup Guidelines being reflected in the Startup Manual. Since

Revision 2 to the Startup Manual eliminated the Startup Guidelines,

this item is irrelevant and considered closed.

g.

(0 pen) Open Item (454/80-23-07):

This item involved the review and

comments on two test procedures.

(1) Diesel Fuel Oil - Essential Service Water, No. 2.24.11 - The

inspector reviewed a draft revision of this test in which the

inspector's comments have been incorporated. This portion of

the. item is considered closed.

(2)

125 Volt DC Distribution, No. 2.21.10 - The inspector obtained

a copy of the issued and completed test procedure. This portion

,

of the item remains open pending completion of the inspector's

review.

(0 pen) Open Item (454/80-23-08):

The licensee plans to prepare

procedures covering housekeeping during preop. These procedures

have not yet been completed. This item remains open pending

inspector review of completed procedures.

In addition, the in-

spector expressed a concern that this area not be neglected since

some equipment has now been tested and turned over to operations.

The licensee acknowledged the inspector's concern.

(Open) Open Item (454/80-23-09): This item involved the preparation

of an administrative procedure to cover control of test and measuring

equipment. The licensee has prepared a draf t procedure for the above.

The inspector reviewed the procedure, BAP.400-4, and the licensee

agreed to include his comments. This item will remain open pending

revision of this procedure.

In a previous inspection, Report No. 50-454/80-23, the licensee

committed to keeping written records of training for preop and test

personnel. The inspector's review of licensee's records revealed

the licensee is now keeping these records.

In addition, the licensee

has formalized his training program requirements into a procedure.

In the same inspection, the licensee indicated that the chemistry

department was developing water chemistry control procedures for

individual plant systems. The licensee is in the process of prepar-

ing these procedures. The inspector reviewed two completed proce-

dures fcr the containment spray and safety injection systems. The

procedures appear to be adequate.

3.

Preoperational Testing Quality Assurance

The licensee's Quality Assurance Program Manual is the manual used by

all the licensee's pl nts includin3 Byron for administering of Quality

Assurance (QA) programs. Organization and responsibilities are speci-

fied in Section Q.R. No. 1 of the Manual.

Requirements for surveillance and audit activities are specified in

Section QR No. 18, Q P. No. 18.1 and No. 18.51 of the QA Manual.

Corrective action and followup requirements for surveillance and audit

nonconformance are specified in Section QP No. 16.1 and No. 18.0 of the

QA Manual.

Review of the above sections of the QA Manual showed the following:

a.

Authorities and responsibilities of individuals managin the QA

program in general have been set forth. The inspectors observed

that the audit procedures are not conducive to the pre-operational

test program in specific areas. This aspect was discussed with

the licensee and it was agreed that the licensee would write an

audit procedure or plan, that would take into consideration those

factors ' hat are peculiar to the pre-operational test program.

Such factors discussed were:

by audit personnel. This is an open item (50-454/81-06-01) pending

development of the procedure.

The Quality ' Assurance Program was reviewed to determine that controls

have been established requiring minimum educational experience, and

qualification requirements for personnel inspecting and auditing the

pre-operational test program.

In addition, the program was reviewed

to determine that responsibilities have been assigned to assure that

pre-operational test requirements will be satisfied.

The licensee's training program for QA auditors and inspectors was

reviewed and determined to include provisions for training on overall

company policies, pracedures and instructions for the QA Program.

The inspectors were unable to verify that the licensee has established

procedores for implementing the QA Program for specific job-related

activities or the specific training requirements for individuals.

These areas will be examined in depth in a later inspection.

This is

an open item (50-454/81-06-02).

4.

Exit Interview

The inspectors met with licensee representatives (denoted in paragraph

1) on June 3, 1981. The inspectors summarized the scope and findings

of the inspection.

The licensee acknowledged the statements by the

inspectors with respect to the open items.
