V NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on Jar.uary 19-23, 19_81, it.
appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below.
Criterion V of Appendix B'to 10 CFR Part 50 states; " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satis-factorily accomplished."
Nonconformances to these requirments are as follows:
A.
QA Manual Section 130.0, paragraph 2, states in part, " Tools, gages, instru-ments, and other measuring and testing devices used in qual-ity verification will be calibrated...." Paragraph 5 states in part, " Measuring or testing devices found to be discrepant at calibration are removed from service...." Paragraph 6 states, "The Quality Assurance Department is responsible for the overall program of identification and calibration...
and for maintaining records of such."
ASME Code Section III, NF-2360 states in part, " Calibration of temperature instruments and C impact test machines used in impact testing shall be performed at the fraquency specified in (a) or (b) below:
(a) Temperature instruments used to control test temperature of specimens shall be calibrated and the results recorded.
. at least once in each 3 month interval.
(b) C impact test machines shall be calibrated and the results recorded V... the calibrations shall be performed at least once in each 1 year interval...."
Contrary to the above:

Records did not exist to substantiate calibration had been performed on temperature instruments or the impact test machine prior to March 1980, even though ASME required C impact testing had been performed at least as early as March, 1978.y (See Details Section, paragraph B.4.a.).

~ Plug Thread Gage No. SP-161 was sent out, calibrated, and returned by a calibration subcontractor on June 6,-1980.
The -certi fication from the subcontractor shows the pitch diameter to be out of tolerance; yet,-this gage was placed back into service and has been used since that time.

QA Manual Section'60.0, paragraph 8 states in part, " Procurement 'of (ASME)
Code materials or services must be from vendors who are approved...."
Paragraph 11 permits the following methods for approval of vendors:
(1)an annual survey by Southern Bolt; or (2) the vendor being in possession of a-valid AS;1E-Quality System Certificate; or (3) the vendor being surveyed and approved by Southern Bolt's customer.
Paragraph NCA-3867.4(e) in Section III of the ASME Code permits a Mater 1al Manufacturer, who certifies stock from a manufacturer whose Quality System Program has not been qualified under NCA-3800, to accept the manufacturer's certification of heat analysis and the requirements of the material speci-fication which must be performed during the melting, provided:

The Material Manufacturer performs or subcontracts all other material specification requirements on each piece of stock material, or, on each heat and lot of material if traceability has been established by his Program or the Prograr of the Certificate Holder who uses the materis1 2.
The Material Manufacturer performs or subcontracts a product analysis to verify the chemical composition of each piece of stock material furnished by the stock material manufacturer.
Contrary to the abovt:
1.
Certain. materials were ased in ASME C de Section III applications, which had beer. received from vendors who had not been surveyed by either Southern Bol: or their customer, and were not holders of ASME Quality System Certificates.
2.
Verification of compliance of these materials with the material specification requirements had not been accomplished in accordance with the provisions of NCA-3867.4(e) in Section III of the ASME Code.
