- On May 27, 1981, Mr. T. D. Geib of Public Service Company of Indiana, Inc.
(PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). Westinghouse had determined that a single random fail-ure in the Volume Control Tank level control system, in the absence of operator action, could possibly lead to a loss of redundancy in the high head safety injection system. The scenario leading up to this event and
.A its potential consequences were described by Westinghouse in their letter (NS-TMA-2451, dated May 21, 1981) to the Director of the Office of Inspec-tion and Enforcement.
In this letter Westinghouse also recommended correcting this situation by assuring that plant procedures properly alert o;erators to take the appro-prince actions to assure an adequate water supply to the charging pumps.
Prior to fuel loading, PSI will review its plant procedures and make any changes necessary to implement this recommendation.
This letter is intended to fulfill the requirements of a final report as defined in 10 CFR SC.55(e).
