Telephone Conversation between J. Eckhardt and B. S. Kaplan on May 21, 1981 (DER 81-13)
Dear Sir:
The NRC was notified of a potentially reportable deficiency in the i
referenced telephone conversation. At that tian, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation required, an interim report is attached. It is now expected that this information will be finalized by October 1,1981, at which time a complete report will be i
submitted.

INTERIM REPORT POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS),
PVNGS UNIT #1 I.
Potential Problem While implementing a Bechtel design change in the 4.16 KV switchgear (tag numbers IEPBAS03 and BSO4) provided by
. General Electric, it was observed that circuit separation requirements had not been maintained as specified by IEEE requirements. The specific concerns are:
a.
The flex conduit which carries Non-Class IE wires
. touches a Class IE wire bundle at the left-hand side of the rear panel.. GE has reviewed this condition and is submitting a Supplier Deviation Disposition
.Raquest (SDDR) to have the conduit relocated to the right, thereby. increasing the spacing.
b.
The SO410t at device "UT" on the rear panel and the Non-Class IE wires (UT 2/XB1 and UT 4/XB2) are not
- separated from the Class IE wire bundle by the re-quired 6".
GE has reviewed this condition and is submitting art SDDR to move device "UT" to right, and to relocate devices "UH" and "XB" as necessary.
II.
Approach To and Status Of Proposed Resolution Bechtel and CE are continuing to investigate this condition to assure that all similar equipment delivered to Palo Verde is reinspected.
III.
Projected Completion of Corrective Action and Submittal Date of the Final Report The evaluation, resolution and final report are forecast to be completed by October 1, 1981.
