s Appendix A NOTICE OF VIOLATION Evart Products Company General License 10 CFR 31.5(c)(5) requires that upon the occurrence or indication of a failure or damage to the shielding of the radioactive material in a device, you shall immediately suspend operation of the device until it has been repaired by the manufacturer or other person authorized to repair such devices.
Contrary to the above, on March 23, 1981, you damaged the shielding of the polonium-210 in a static eliminator and then continued operation of the device until March 26, 1981.
This is a Severity Level V violation (Supplement VII).
No reply to this ites of noncompliance is required since corrective action was completed at the time of the inspection.
