Yin of this office on April 8-9, and 13-15, 1981, of activities at the Clinton Power Station, Unit 1, authorized by NRC Construction Permit No. CPPR-137 and to the discussion of our findings with Messrs. L. J. Koch, J. O. McHood, and others of your staff at the conclusion of the inspection. The enclosed copy of our inspection report identifies areas examined during the inspection. Within these areas, the inspection consisted of a selective examination of procedures and representative records, observations, and in- terviews with personnel. As a result of our inspection conducted in February and March 1981, (Region III Report No. 50-461/81-05) you stopped installation of safety related piping suspension systems. We confirmed the stop work with an Immediate Action Letter dated March 5, 1981. During this followup inspection additional examples of failure to follow procedures for installation and inspection of large bore piping suspension systems were identified. Our inspector discussed the deficiencies with your technical staff, and it was agreed that a second trial program should be initiated. Further, it was agreed that your stop work order will remain in effect until completion of the following additional corrective actions: 1. Conduct a comprehensive evaluation of existing hanger welding and QC inspection activities. 2. Conduct further training of personnel on the latest program revisions. 81061500 6 Q
-- a Illinois Power Company -2- MAy g gg 3. Limit your second trial program to inspection of 100 installed hangers, and installation of 100 new hangers. Your sample should be selected from hangers which are complex in configuration, i.e., those fabricated from several structural members and involving multiple welds and/or other joining methods. 4. Notify this office when you have completed the installation and inspection of 50 new hangers, the inspection of the 50 old hangers, and the documenta- tion of surveillance and audits for this part of the second trial program. Since these actions were reviewed with your staff, no additional response is necessa ry. We will examine these actions during subsequent inspections. In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the en- closed inspection report will be placed in the NRC's Public Document Room, except as follows. If this report contains information that you or your contractors believe to be proprietary, you must apply in writing to this office, within twenty-five days of the date of this letter, to withhold such information from public disclosure. The application must include a full statement of the reasons for which the information is considered proprietary, and should be prepared so that proprietary information iden- tified in the application is contained in an enclosure to the application. We will gladly discuss any questions you have concerning this inspection.
