g APPENDIX A NOTICE OF VIOLATION Kansas Gas and Electric Company Docket: STN 50-482 Wolf Creek, Unit 1 As a result of investigation and inspection activity conducted during the month of April 1981, the following violation was identified.

10 CFR Part 50, Appendix B Criterion II recuires in part that the Quality Assurance Program shall provide control over activities affecting quality of structures, systems and components.

Criterion VIII requires in part that identification and control

?.easures shall be designed to prevent the use of incorrect or defective material, parts and components.

Criterion XV requires in part that measures shall be established to control materials, parts, or components which do not conform to require-ments in order to prevent their inadvertent use or installation.

Centrary to the above, an unauthorized piping hanger was fabricated fran non-confonning materials to replace an approved safety-related piping hanger (BN 01 C011 123) in the Borated Refueling Water Storage system which was inadvertently destroyed.

Fabrication of the unauthorized hanger constituted a breakdown in the licensee's Quality Assurance Program in that the activity was conducted in deliberate disregard of the licensee's program for control over activities affecting quality that resulted in bypassing of the requirement for prevention of use of innrrect material and failure to provide control of materials which do not '.Jnfonn to requirements.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Kansas Gas and Electric Company is hereby required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps 8106120 33(p

o Kansas Gas and Electric Company Wolf Creek, Unit 1 which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affinnation.

Consideration may be given to extending your_ response time for good cause shown.
