APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Docket No. 50-327 Sequoyah 1 License No. OPR-77 As a result of the inspection conducted on February 1 - March 5,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violation was identified.
Technical Specification 3.6.1.1 requires th'at containment integrity shall be maintained in modes 1, 2, 3, and 4 and that containment penetration
, isolation be verified periodically. Containment integrity is based in part on 10 CFR 50, Appendix A, Criterion 56, which requires that lines that penetrate containment and are open to the containment atmosphere be isolated by one locked closed valve inside and one locked closed valve outside containment.
Contrary to the above, containment integrity has not been adequately maintained or verified in that, position requirements for the inside con-tainment isolation valve for the service air system, vaive 1-33-704, were not included in system operating instruction SOI-88.1, Containment Isolation System, or in s,urveillanco instruction SI-14, Verification of Containment-Integrity.
Valve 1-33-704 was normally maintained in the open pusition however, the outside containment isol.ation valve, 1-33-740, was maintained closed and periodically ve'rified to be in the proper position during unit operation.
This is a Severity Level IV Violation (Supplement I.D.3.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within twenty-five days of the date of this Notice, a written state-ment or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the correctine steps which have been taken and the results achieved; (4) corrective steps which wil1 be taken to avoid further violations; and (5) the date when full compliance j
will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
