g (p In accordance with the requirements of Title 10, Chapter 1, Code of Federal Regulations, Part 21, Transamerica Eelaval, Inc. , hereby notifies the commission of a potential defect in a component of a DSRV-16 Standby Piesel Generator. There exists a potential problem with oil plugs in the c ankshaft, which could result in engine non-availability.

The only engine which Transamerica Delaval has supplied with the potential defect was to the Tennessee Valley Authority, Phipps Bend Station, Unit 2, engine serial #77034-2932. This engine was shipped in May, 1981.

The crankshaf- for this engine was manufactured by Transamerica Delaval at Oakland, California. The oil plugs which real drilled oil pcssages are installed by Iransamerica Delaval.

The potential defect or the oil plug was caused by improper installation.

This plug is rolled into the crankshaft using an expansien tool. The plug is a slight drive fit in che crankshaf*. It was driven into the shaft using a steel tool. The tool used on this engine was undersize.

The mechanic used excessive force during installation causing an upset on the bottom of the plug. This upset could lead to failure of the plug.

To eliminate this problem, a new hardwood driver is now used.

The existence of the upset can be determined by visual i.ispection. If the defective plugs are not removed and replaced, engine availability could be affected.

A copy of this letter will be sent to Tennessee 7 alley Authority. The engine in questien is presently in storage at TVA. ~4e will gi're TVA all the information cnd material necessary to eliminate this problem in accor-dance with our contract. This information will be sent by July 31, 1981.
