9 APPENDIX A NOTICE OF VIOLATION I

South Carolina Electric and Gas Docket No. 50-395 V. C. Summer License No. CPPR-94 As a result of the inspection conducted on February 12, - March 31,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),

t the following violation was identified.

10 CFR 50 Appendix B, Criterion V, as implemented by FSAR Section 17.1.5 of the FSAR requires that activities affecting quality be prescribed by documented instructions, procedures or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instruc-tions procedures or drawings.

Quality Control Procedure VII-13 " Quality Verification of Hydrostatic and Pneumatic Tests," Section 4.6.6 requires that all valves within the test boundary be properly aligned so that all components will be subjected to test pressure.

Contrary to the above, on January 27, 181 Hydrostatic Test CS-21-H1, Holdup Tank, was performed and subsequently approved with valve 8620 shut, preventing the piping between valve 8620 and 8621 (Hydrostatic Test Boundary) from being subjected to test pressure.

This is a Severity Level Violation V (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to r

this office within twenty-five days of the date of this Notice, a written state-ment or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which I

will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
